CLINICAL PRECEPTOR NURSE JOB SUMMARY

The Clinical Preceptor is responsible for the in-home orientation of new clinical staff to Home Health Care. This position utilizes standards for practice to teach clinical staff the safe delivery of high-quality care to patients in accordance with Agency Policies and Procedures, State, Federal and CHAP Regulations. Ensure quality and safe delivery of nursing and home health services rendered. Coordinate provision of Home Health Care Services that reflect the agency philosophy and standards of home health and nursing care of assigned clients.

 

CLINICAL PRECEPTOR NURSE ESSENTIAL DUTIES & RESPONSIBILITIES

  • Responsible for the in-home clinical orientation of new clinical employees by providing one-on-one teaching and modeling of job duties.
  • Demonstrates knowledge, comprehension, and willingness to comply with all job functions of an RN Case Manager.
  • Demonstrates knowledge, comprehension, and compliance with all state and federal laws, rules and regulations, and agency policies and procedures, as well as the ability to enforce them.
  • Demonstrates a sincere desire to continually improve and upgrade the skills and knowledge of the RN Case Managers through the provision of initial and continuing education.
  • Demonstrates the ability to accurately complete and review all documentation as relates to the provision of care of patients or in daily operations and to instruct others in its proper completion.
  • Evaluate and document skill performance of preceptee during competency verifications, both in a classroom and in the field.
  • Responsible for the assessment of skills and competency of new clinical staff through direct observation. Observe and review technique and provide constructive feedback when appropriate.
  • Follow new clinical employees through the orientation period (initial 90 days) in order to provide training,mentoring and clear direction.
  • While precepting, manage a reduced case load and perform home visits with staff for the purpose of role modeling, problem solving, supervising and evaluating performance/quality of care. During periods when not precepting, position functions as an RN Case Manager.
  • Demonstrate timely and accurate computer entry of patient information into the CERNER system to facilitate the development of the OASIS-C data set, orders for service, creation of time sheets and other required patient / employment documents and reports.
  • Conduct patient clinical record reviews to assess the appropriateness of care plan, the utilization of services and to ensure complete documentation of needed services provided by new clinical staff.
  • Participate in the orientation of staff members to the Agency and its Policies and Procedures.
  • Participate in program planning and evaluation activities related to the clinical services of the Agency.
  • Answer questions concerning all aspects of patient care.

 

EDUCATION & QUALIFICATION REQUIREMENTS

  • A Baccalaureate Degree from an accredited School of Nursing and a minimum of two (2) years of fulltime clinical experience within the past five (5) years in a home care agency or related health care facility; OR an Associate Degree in Nursing from an accredited School of Nursing and a minimum of three (3) years of full-time clinical experience, with two (2) of these years being in a home care related facility/program which included care of the sick, and evidence of certification by the American Nurses’ Association as a community health nurse or completion of at least six (6) credits within two (2) years in community health nursing theory or six (6) credits in health care management from an accredited college or university program or school of nursing; OR a Diploma in Nursing from an accredited School of Nursing and at least three (3) years of fulltime clinical experience in nursing within the past five (5) years, of which two (2) were in a home health agency or health care facility/program.
  • Current license / registration by state of hire as a Registered Nurse.
  • Knowledge of State Regulations, Medicare, Medicaid and CHAP Guidelines is desirable.
  • Graduate of an accredited School of Nursing, College or University.
  • Excellent interpersonal, communication and organizational skills.
  • Ability to relate constructive criticism and mentor new employees.
  • Ability to motivate co-workers and subordinates to project professional attributes.
