CLINICAL MANAGER JOB SUMMARY

The Clinical manager supervises the provision of skilled services provided by the team and contributes to the overall company success. The Clinical manager effectively manages the team’s clinical operations, services, personnel performance, and office management on an ongoing basis.

 

CLINICAL MANAGER ESSENTIAL DUTIES & RESPONSIBILITIES

  • Ensure that the client’s plan of care is individualized and appropriate, executed as written and reassessed by the appropriate health care professional when there is a significant health status change in the client’s condition, at the physician’s request and after hospital discharge. Ensure appropriate documentation is completed for all patients transferred to an inpatient facility.
  • Complete review of evaluation packets, OASIS and 485s. Review both the data submitted via the Mobile Device and via paper to ensure accuracy and follow up on any documentation that requires correction. Process the OASIS and 485, verify the correct start of care date and episode date range in HCHB. Process any unlisted supplies, medications, activity, functional limitations, allergies, etc. that appear on the action screen.
  • Follow up on OASIS Assessments that cannot be processed due to Licensed Professional documentation deficiencies.
  • Monitor HCHB action screens daily.
  • Ensure generation of monthly on-call schedule and review of on-call log on a daily basis.
  • Particpate incase conferences and address recertification/discharge decisions based on Case Conference findings.
  • Complete chart audits, document completion and maintain in appropriate log.
  • Take administrative on-call as a support to the on-call nurse and to provide software management related to processing intakes, reviewing evaluation packets, and scheduling, moving, reassigning visits to ensure the smooth function of the Agency during off hours.
  • Ensure that end of episode billing is completed timely and outstanding claims are kept to a minimum.
  • Ensure deficiencies related to the Central Support Office audit findings are addressed and corrected per action plans. Maintain Clinical Audit scores as per established benchmarks.
  • Ensure productivity standards are met for full time and part time employees.
  • Review Infection Control reports for all clients. Provides all follow up related to Infection Control reports (both client and employee) and processes these reports in Homecare Homebase.
  • Review Occurrence Reports. Provide all follow up related to Occurrence Reports and processes in Homecare Homebase.
  • Conduct Clinical Quality Improvement (CQI) functions in collaboration with the DPS.
  • Participate in quarterly review of statistics of the Clinical Chart Review.
  • Implement and manage corrective plans congruent with Policies and Procedures.
  • ADR and Denial review, preparation and submission per established protocols.
  • Work closely with the Coding Department. Ensure that the OASIS scoring is correct. Validate the frequency of visits based on assessment scoring.
  • Correct rejected OASIS as per the Validation Report per OASIS batch submission.
  • Participate in the Agency Disaster Plan including, but not limited to: 1. Committee to annually review the disaster preparedness plan 2. Test the call tree per policy, present/perform a disaster drill annually 3. Activate the emergency preparedness plan when a disaster is imminent in the absence of the Administrator 4. Activate the Emergency Preparedness and Response Plan for clients 5. Supervise and direct the emergency preparedness plan implementation 6. Act as the Agency Disaster Coordinator in the absence of the Branch Administrator.
  • Participate in community awareness and education programs.

 

EDUCATION & QUALIFICATION REQUIREMENTS

  • Bachelor’s of Science in Nursing (BSN) and two years’ experience as a supervising community health nurse, OR, four years experience as a certified home care nurse.
  • Currently licensed and registered to practice as an RN with the State Board of Nursing.
  • Minimum of 1 year previous supervisory/management experience
  • Knowledge of State and Federal Home Care Regulations
  • Must be capable of multitasking while maintaining a professional and friendly demeanor and be able to function well in an atmosphere of stress and interruption.
  • Demonstrates outstanding leadership skills and interpersonal skills.
  • Must be organized and able to effectively communicate both orally and in writing.
